The rating provided is somewhat unfair consider I have yet to admit; but am happy to rectify it upon conclusion the surgery is successful. However at this point in time, my initial thoughts are that the contents of the General Admission Form was ridiculously technical and filled with so much medical jargon I may as well have gone through a medical school crash course. The few words I could pick out such as Retinal/Optic Nerve could easily be simplified to layman terms such as "Eyes" or "Eye Region" with an option to specify if the patient knows the technical term. The benchmark is clearly set too high and makes me wonder how the elderly or non English speakers are supposed to fill this form out moreso if they dont have children who are able to help with the complexity? However, I commend the group for creating a rather easy to use and user friendly e-Form but the content remains too complicated and recommend the group make further attempts at simplification targeting the actual questions and various technical content.
